A 45-year-old female, obese with a BMI of 39.5 kg/m2 was scheduled to undergo hysteroscopy for endometrial hyperplasia under spinal anesthesia. In the operating theater, in sitting position, the spinous process was difficult to detect and so the line of iliac crests as landmark for the level L4-L5 was used for puncture.
