English abstract

We here report the case of a 53-year old patient, presenting with dry cough and dyspnea on exertion. Chest scanner showed bilateral mediastinal and hilar lymphadenopathies and left interlobular lymphadenopathy associated with bilateral strip and subsegmental ventral, lingular and basal atelectasis. Bronchoscopy showed diffuse inflammation and right in catheterizable spur thickening middle lobe bronchus with scalloped appearance and vascular smooth bud bleeding at the slightest contact at the level of the right posterior-basal segmental bronchus. On the left side , there was a highly vascularized bud at the level of the left posterior-basal bronchus which was smaller than that of the right bud but more inflamed. Biopsy showed granulomatous alterations confirming the diagnosis of sarcoidosis.

Patiente de 53 ans, consulte pour une toux sèche et une dyspnée d’effort. Le scanner thoracique montre des adénopathies médiastino-hilaires bilatérales et interlobulaires gauches avecdes atélectasies en bandes et sous segmentaires ventrales bilatérales, lingulaire et basales bilatérales. La bronchoscopie montre un aspect inflammatoire diffus et un aspect festonné de la bronche lobaire moyenne droite qui est incathéterisable avec des éperons épaissies et un bourgeon vascularisé et lisse saignant au moindre contact au niveau de la bronche segmentaire postéro-basale droite. A gauche, on trouve un bourgeon très vascularise au niveau de la bronche postero-basale gauche de taille plus petite que celle du bourgeon droit mais il est plus inflammatoire. La biopsie montre des remaniements granulomateux en rapports avec une sarcoïdose.
